Passport – with SIGNATURE on it

PRC LERIS online account credentials (username and pw). If wala, create muna po kayo

Authorization letter
REPRESENTATIVE NAME: EDGAR CORPUZ JR

Please specify where the soft copy should be emailed to i.e. AHPRA, ANMAC, etc.
