Passport – with SIGNATURE on it

PH Departure stamp in your passport

Old Flight Ticket / Boarding Pass

Authorization letter
REPRESENTATIVE NAME: MICHAEL DOLLESEN

𝙄𝙈𝙋𝙊𝙍𝙏𝘼𝙉𝙏 𝘾𝙊𝙉𝘿𝙄𝙏𝙄𝙊𝙉𝙎
1. LTO will only issue an official receipt (wala po new ID).
2. No more than 2 yrs expired.
3. Can only renew no more than 2 mos before expiry (not too early).
4. Kapag umuwi na po kayo ng Pinas, kailangan po within 30 days upon arrival mag report kayo sa LTO Main branch (East Ave QC) for photo-taking and biometrics for your new ID.
